New Initiative Expands Cardiovascular Screening in Community Health Centers

A new initiative aims to enhance screening for elevated lipoprotein(a), commonly known as Lp(a), in community health centers (CHCs) across the United States. This project, spearheaded by the American Heart Association with support from Novartis Pharmaceuticals Corporation, seeks to raise awareness about Lp(a) as a significant risk factor for cardiovascular disease (CVD), which remains the leading cause of death globally.
Elevated Lp(a) is an inherited condition that contributes to cardiovascular disease independent of lifestyle choices. Similar to low-density lipoprotein (LDL), or “bad” cholesterol, Lp(a) levels are primarily determined by genetics. Research indicates that approximately 1 in 5 Americans may have high Lp(a) levels, yet there is a troubling lack of awareness regarding this risk. A recent study reveals that fewer than 1% of individuals with atherosclerotic cardiovascular disease (ASCVD) undergo testing for Lp(a).
Expanding Awareness and Access
The Lp(a) Community Health Centers Discovery Project aims to improve both awareness and screening processes related to Lp(a). It emphasizes the importance of shared decision-making between healthcare providers and patients regarding ASCVD risk management. According to Kaavya Paruchuri, M.D., a volunteer with the American Heart Association and director of the Lipid Apheresis Unit at Massachusetts General Hospital, “A simple, one-time Lp(a) test can provide lifelong insight about overall heart disease risk—especially for individuals with a family history of heart conditions.”
As of 2024, 10 community health centers have joined this initiative, with plans to add another 10 sites this year. Participating centers have successfully integrated new Lp(a) screening processes, resulting in increased testing rates. Notable CHCs involved include:
– Bay Area Community Health in Fremont, California
– Heart of Ohio Family Health Centers in Columbus, Ohio
– El Centro Family Health in Española, New Mexico
This initiative builds on the findings from the 2023 Lp(a) Discovery Project, which gathered insights from various health systems to identify effective Lp(a) testing methods. The lessons learned from these implementations will be pivotal in refining practices across the newly participating CHCs.
Addressing a Critical Gap in Awareness
Despite the significant health implications associated with elevated Lp(a), a baseline survey conducted as part of the Discovery Project found that few U.S. adults are familiar with it. This lack of awareness is coupled with a general absence of urgency surrounding the need for testing. Many individuals do not associate elevated Lp(a) levels with cardiovascular risk; however, when they do recognize this connection, they are more motivated to engage their healthcare providers.
Healthcare professionals play a crucial role in raising awareness about Lp(a). Their recommendations and personal experiences are essential in encouraging patients to consider getting their levels checked.

This project represents a significant step toward improving cardiovascular health outcomes by increasing access to vital screening tools in underserved communities. By focusing on education and proactive health measures, the American Heart Association aims to empower individuals to take charge of their heart health and reduce the risk of cardiovascular disease.
