During a signing ceremony for First Lady Melania Trump’s initiative, “Fostering the Future for American Children and Families,” Trump repeatedly substituted the word “adopt” for “adapt.” Critics have seized on this incident, claiming it reflects a decline in his mental capabilities.
Video footage from the event shows Trump mixing up his words several times. He stated, “Christians and more, think of this, more than twice as likely foster care, they’ll adopt the general population.” This confusion, coupled with an apparent difficulty in clapping, has led many to express concern over his cognitive state. Social media users quickly reacted, with one remarking, “He’s getting worse by the day,” while another harshly compared his mental state to “a bag of garbage left out in the sun.”

Analysts clarified that this assessment was likely the Montreal Cognitive Assessment (MoCA), a tool used to identify early signs of dementia, rather than a measure of intelligence.
These moments have intensified calls from critics for the invocation of the 25th Amendment, citing concerns about Trump’s fitness for office. Despite the growing public unease, the White House has remained steadfast in its position. A note from the White House physician, Sean Barbabella, in April 2025, asserted that President Trump “exhibits excellent cognitive and physical health” and is fully capable of fulfilling his duties.
White House Response and the Ongoing Debate
A follow-up memo issued in October reaffirmed Trump’s “exceptional neurological and physical performance,” stating he again scored perfectly on cognitive testing. Leavitt has indicated that the administration plans to release medical information “as soon as possible.” Nevertheless, no official statement has addressed the specific incidents of word confusion or difficulty clapping.
As public scrutiny of Trump’s cognitive abilities continues, clinical experts caution that while word confusion and motor hesitation may indicate potential early cognitive decline, these signs alone are not definitive without comprehensive neurological assessments. The debate surrounding Trump’s mental health is likely to persist, fueled by viral footage and a lack of direct responses from the White House regarding these incidents.
In the meantime, the scrutiny surrounding Trump’s presidency remains intense as he navigates the challenges of public perception at an age when many are facing significant health questions.
