Highland GP Raises Concerns Over Walk-In Clinic Plans

A proposal to establish a network of walk-in GP services across Scotland has prompted concerns from a prominent Highland general practitioner. First Minister John Swinney announced the initiative at the SNP conference last weekend, aiming to ease the demand for doctor appointments during morning hours and enhance healthcare accessibility.
The plan outlines the introduction of walk-in clinics located in high-traffic areas, such as high streets, schools, and workplaces. These facilities are set to operate from noon to 20:00 seven days a week, allowing patients to access care without prior appointments. Swinney stated that the initiative would begin with 15 sites across Scotland, with a goal of providing one million additional GP and nurse appointments within a year.
While the initiative aims to alleviate pressure on traditional GP services, not everyone is convinced of its efficacy. Dr. Chris Williams, vice chairman of the Royal College of General Practitioners (RCGP) Scotland, expressed concern that the proposed approach could lead to “fragmented” care. He emphasized that it may not significantly relieve the existing demands on core GP services or reduce the number of patients visiting accident and emergency departments.
Dr. Williams, a GP based in Grantown-on-Spey and a whistleblower on bullying within NHS Highland, raised specific concerns regarding the implementation of the walk-in services in rural areas. He noted that the locations for these clinics have yet to be determined, and there is a possibility that a remote or rural site could be chosen as one of the pilot locations.
“While the concept of walk-in services may seem beneficial,” he said, “evidence from other regions indicates that this model can lead to fragmented care. It does not necessarily reduce the strain on core GP services or decrease emergency department visits.”
In addition to the potential fragmentation of care, Dr. Williams pointed out that NHS Highland already provides Investigation and Treatment Room (ITR) services. These services are delivered by community nursing teams and enable patients to receive tests requested by secondary care specialists closer to their homes. As the walk-in primary care pilot progresses, he emphasized the importance of ensuring that residents in remote and rural communities are not disadvantaged.
“Patients should not be expected to travel long distances for necessary care,” he added, highlighting existing challenges in recruiting healthcare staff in rural regions.
In response to these concerns, Neil Gray, Scotland’s Health Secretary, emphasized the government’s commitment to improving access to primary care. He stated, “Shifting more care into the community is central to our approach. We are pursuing innovative solutions to enhance primary care access, including local walk-in GP clinics.”
Gray assured that these clinics would complement core GP services and offer patients additional flexibility. He noted that their development would involve collaboration with NHS boards, general practitioners, and other stakeholders to ensure they strengthen existing services.
As the Scottish government moves forward with this initiative, the effectiveness and impact of walk-in clinics on the overall healthcare system remain to be seen, particularly in rural areas like the Highlands. The debate continues as stakeholders evaluate how best to balance accessibility with the quality and continuity of care.
