Mental Health Clinicians Share Insights on Patient Loss Impact

Two leading mental health professionals from New Craigs, the only psychiatric hospital in the Highlands, have opened up about the profound impact that patient loss has on staff. In a rare candid discussion, Dr. Neil McNamara, the clinical director of mental health services, and Jonathan Davies, the associate lead nurse for mental health, highlighted the emotional toll faced by clinicians when a patient takes their own life.
The conversation was prompted by the tragic case of Will Ogilvy, who ended his life in 2024. His mother, Jacqui Waring, had fought for changes within the health board to improve mental health care following her son’s death. Diagnosed with schizophrenia and sectioned for his own safety, Ogilvy’s story underscores the complexities and challenges faced by mental health professionals.
Understanding the Emotional Burden
In their discussion, both Dr. McNamara and Mr. Davies addressed the often-overlooked emotional burden borne by mental health staff. “What we do is mainly done through therapeutic relationships,” Mr. Davies explained. “Our whole person goes into battle with our patient against their illness or crisis.” This deep connection can make the loss of a patient feel far more significant than in other medical fields, where interactions tend to be shorter and less personal.
“When someone dies, the impact on our staff can be quite substantial,” he added. Clinicians frequently engage closely with patients and their families, making the emotional repercussions of a loss deeply felt. “It can be very difficult for staff when we lose someone,” Mr. Davies said, emphasizing the need for support within the mental health community.
Dr. McNamara echoed these sentiments, describing experiences of loss as both a shock and a tragedy. “There will be a sadness for what has been lost and missed opportunities,” he noted. “Additionally, families will share in this experience, often feeling a terrible sense of guilt.”
Professional and Personal Reflections
Both clinicians acknowledged that the emotional responses to patient loss are a natural part of their work. “You go through this process of questioning yourself about whether you did everything you could have done,” Dr. McNamara stated. This introspection can weigh heavily on mental health professionals, affecting their well-being and requiring additional support.
Despite the emotional toll, Dr. McNamara pointed out the importance of maintaining professional obligations. “We recognize the human impact of this but also understand our responsibilities,” he said. The team is tasked with reflecting on each situation to determine whether there were predictable factors or missed opportunities for intervention.
This soul-searching not only aids staff in processing their grief but also serves to enhance mental health care practices. The collaboration with Ms. Waring, who sought to implement changes following her son’s death, exemplifies the positive outcomes that can stem from such tragedies. “I’m grateful to her for working with NHS Highland,” Dr. McNamara remarked, praising her courage and the importance of breaking down stigma surrounding mental health issues.
“Engaging with families right from the start can be powerful,” he added. “We know how valuable these conversations are, and while we don’t always manage it, we are continually striving to improve.”
The reflections from Dr. McNamara and Mr. Davies highlight the critical need for support systems within mental health services. Their insights reveal the often hidden struggles faced by clinicians and the importance of addressing both professional responsibilities and personal emotional health in the wake of patient loss.
